Home Health Services
Larksfield Place wants seniors to maintain their independent lifestyles. With home health care, you’ll receive personal assistance from people you know and trust. Their staff are there for you, providing “hands-on” care, whether you need help with the simple necessities of life such as bathing, dressing, medication reminders or meal preparation, or if you need services from a professional nurse such as taking medications or changing a dressing from a recent procedure. The goal of Larksfield Place is to make it easier for you to maintain an independent lifestyle while seeing to your healthcare needs.
Transitional Care
Larksfield Place Assisted Living offers an option for those who have recently been discharged from the hospital or skilled nursing care, and are not quite ready to return home. This collaboration of care and services is designed to achieve quality outcomes, a positive patient experience and reduce unnecessary readmissions to the hospital. Available 24/7, allowing for less than a 30-day stay. The costs are much less expensive than private duty nursing.
Tammy Flaming, Director of Marketing says, “Our services assist people in recovery so they can safely return home. A huge component of the short-term stay is therapy. Larksfield staff physical therapists, occupational therapists and speech-language pathologists help patients reach their goals.”
